Members select a per-person deductible between $3000, $5000, $7500, or $10,000, depending on the plan they select. Eligible plans’ types of deductibles may be changed from “per cause” to “per term,” offering one amount to meet according to the plan’s coverage period and fewer out-of-pocket medical costs.

Your portion of coinsurance is either 30 percent or 20 percent with a maximum of $10,000. Primary Care and Speciality Doctor Office Visits (Including Urgent Care) are subject to deductible and co-insurance.

Members select a per-person deductible of $2000, $3000 or $5,000, depending on the plan they select. Eligible plans’ types of deductibles may be changed from “per cause” to “per term,” offering one amount to meet according to the plan’s coverage period and fewer out-of-pocket medical costs.

Your portion of coinsurance is 20 percent with a maximum of $5,000. Primary Care and Speciality Doctor Office Visits (Including Urgent Care) are subject to deductible and co-insurance.

Prescription drugs on this plan are discounted and after a $500 Rx Deductible provide a generic copay of $10, preferred brand copay of $50, and a non-preferred brand copay of $75. Speciality drugs are not covered under this plan.

These Pivot Health short-term plans limit the maximum coverage period benefit of $500,000 or $1 Million depending on the plan you select. These temporary plans are offered with various benefits and options, depending on your state.

Members select a per-person deductible between $1000, $2500, or $5000, depending on the plan they select. Eligible plans’ types of deductibles may be changed from “per cause” to “per term,” offering one amount to meet according to the plan’s coverage period and fewer out-of-pocket medical costs.

Your portion of coinsurance is 20 percent with a maximum of $3,000. Primary Care Visits have a $30 copay. Urgent Care and Speciality Doctor visits have a $60 copay.
